1.  Tell them you want to participate. We will then contact your Doctor to get approval
for you to participate in this program.

2.  If authorized by your Doctor, they will send you an A1c test kit to your home. You
then perform the simple finger stick A1C test and return the sample to our lab with the
included prepaid envelope.

3.  FlexSite analyzes the sample and the results will be sent to you and your Doctor.

4.  Then, depending upon the frequency that your Doctor orders the test, you will
receive  the next test in approximately 3 months*.

*The American Diabetes Association recommends that patients with diabetes have the important A1c test
performed up to 4 times per year.
